Completing Your Eagle Scout
Rank Application

Because of the significance of the Eagle Scout rank, the Eagle Scout Rank Application must be accurately completed. Failure to do so may result in a delay in awarding of the Eagle Scout rank.

By completing the application using the following supplemental instructions, you will have a greater chance of your Eagle Scout Rank Application being accepted on the first submission.

Obtaining the application:

First, ensure you have a current application: BSA No 58-728; this is a one sheet, 2-sided application and should have a date of 2000 or later.

If you do not have an application, you can get one from the Spokane Service Center, or online at the BSA National website. Do not make photocopies of the application form! The form you submit must be an original copy!

NOTE: If you download the form, be sure to print it on one piece of paper. This will eliminate the possibly of having the second page becoming lost.

You will need the Adobe Reader to view/print the form. If you do not have the Adobe free reader, you can get it by clicking on the following graphic:

Now that you have a current application, place these supplemental instructions beside the form while you are completing the form. (Red words are references to the RED TEXT on the application form.)

PID NO. Although this says required, LEAVE IT BLANK! The council office will make an entry here.

LIST YOUR FULL LEGAL NAME (UP TO 30 CHARACTERS ONLY) List your name, entering one character or punctuation mark per block, the way you wish it to appear on your Eagle Scout certificate. Be sure to leave a blank space between your first name, middle name or initial, and your last name.

SOCIAL SECURITY NO. One number per block; be sure this is correct!!!

Street or R.F.D. Address Ensure your mailing address is listed here; if you receive your mail at a PO Box in lieu of a street address, enter the words "PO Box" (without the quotes) followed by the box number.

City, State, Zip and Telephone (Including area code) These are self explanatory; however, ensure they are legible!

Troop, Team, Crew, or Ship Indicate whether you are a member of a Troop, Team, Crew, or Ship by legibly printing the appropriate word.

Local No. This is your unit number.

City, State, Zip This is the location where your unit is chartered. This must match exactly what is on your unit's charter so if you are in doubt - ASK!!!

For each of the following items, ensure you enter the dates correctly:

Date joined a Boy Scout troop
Date became a Varsity Scout
Date became a Venturer
Date of First Class Scout board of review
Date of Star Scout board of review

Dates must be in the form of mm dd yy (month day year - two digits each). Months and days before the 10th should be written with a zero in the first position of the month or date block, i.e. 01 or 07.

Be sure you answer Yes or No for each of the following questions:

Were you a Cub Scout?
Were you a Webelos Scout?
Did you earn the Arrow of Light Award?
Had you completed fifth grade upon joining?

AGE REQUIREMENT ELIGIBILITY. Be sure you understand this paragraph. If in doubt, ask your unit leader. Waivers are not granted if you do not meet the age requirements!

Date of birth Enter your date of birth using 2 numbers. If your month and/or date is before the 10th, place a zero in the first block, i.e. 01 or 07.

REQUIREMENT 1. Enter the date of your Life Scout board of review using 2 numbers for the month day and year. Remember, if your month and/or date is before the 10th, place a zero in the first block, i.e. 01 or 07.

REQUIREMENT 2. The entries in this section must match your letters of reference. You must have a minimum of three letters! For those lines that you do not have letters for, enter "NOT APPLICABLE" (without the quotes).

REQUIREMENT 3. Note the star and it's accompanying remarks for merit badges 6 and 9. If more than one of the listed merit badges has been earned, cross out all but one merit badge; list any of the crossed-out merit badges on lines 13 thru 21.

NOTE 1: ALL merit badges earned since becoming a Scout may be listed, not just those the Scout has earned since First Class.

NOTE 2: Dates must be in the form of mm dd yy (month day year - two digits each). Months and days before the 10th should be written with a zero in the first position of the month or date block, i.e. 01 or 07.

NOTE 3: Enter only the unit number of the unit you were a member of when you earned the merit badge.

REQUIREMENT 4. For this requirement, ensure you list the position as it is listed on the application. Remember, dates must be in the form of mm dd yy (month day year - two digits each). Months and days before the 10th should be written with a zero in the first position of the month or date block, i.e. 01 or 07.
Ensure the total time listed covers ANY six month period since the Life board of review. If the individual served in more than two positions of responsibility, list them on a separate sheet of paper (also place the Scout's name on the top of the separate sheet of paper and the words "Requirement 4 continued" (without the quotes); this will ensure proper credit).

NOTE: The date you enter for the Date of Life Scout Board of Review here must match exactly the date you entered for requirement 1!

REQUIREMENT 5. For this requirement, follow the instructions on this page of this website.
Remember, you must use the Eagle Scout Leadership Service Project Workbook, No. 18-927, in meeting this requirement. If you do not have this item, you can obtain a copy from the Spokane Service Center or by downloading a copy from the BSA National website.

NOTE: Here you have a choice of downloading a PDF copy (which can be printed, then completed by hand) or you can download an RTF copy (which can be completed on your computer; virtually all word processing applications are capable of opening a Rich Text Format document!).

REQUIREMENT 6. "What should I say here?" is a common question. The key here is the same as in the unit leader conferences for each rank award. DISCUSS it with your unit leader. Before you write it down, organize your thoughts. Remember, this must be your words, not your unit leader's words! While this can be hand written, it is better to use a computer to complete this document so you can check your spelling and grammar! Be sure to review the Eagle Scout Rank Application to ensure you include everything that is required!

Date conference was held. This is the date you and your unit leader discuss your life purpose statement. Remember that dates must be in the form of mm dd yy (month day year - two digits each). Months and days before the 10th should be written with a zero in the first position of the month or date block, i.e. 01 or 07.

After you have completed and rechecked all the above requirements; you can sign the application; enter your complete telephone number, including area code.

The date entered here is the date you sign this application. Remember that dates must be in the form of mm dd yy (month day year - two digits each). Months and days before the 10th should be written with a zero in the first position of the month or date block, i.e. 01 or 07.

You are now ready to give the completed Eagle Scout Rank Application to your unit leader. But, before doing so, take a look at the following common errors or omissions typically found on the Eagle Scout Rank Application.
  1. Dates:
    Birth date missing or incorrect
    Must have four months between First Class and Star Scout Awards
    Must have six months between Star Scout and Life Scout Awards (date of Life Scout board of review missing?)
    Must have six months between Life Scout Award and Eagle Scout Board of Review (date of Life Scout board of review missing?)
    Dates for merit badge(s) missing
    Eagle Scout Board of Review date missing

  2. Required Merit Badges:
    Four required merit badges must be earned prior to Star Board of Review
    Seven required merit badges must be earned prior to Life Board of Review
    Non-existent or incorrect names of merit badges listed

  3. Position(s) of Responsibility:
    Must serve actively for a period of 6 months in a valid position between Life Scout and Eagle Scout Board of Review dates (inclusive dates of position(s) of responsibility not shown or do not total six months)

    Qualifying Positions of Responsibility:
    • Boy Scout troop:
      Patrol leader, assistant senior patrol leader, senior patrol leader, troop guide, Order of the Arrow troop representative, den chief, scribe, librarian, historian, quartermaster, junior assistant Scoutmaster, chaplain aide, or instructor
      Notice assistant patrol leader is not a qualifying position!

    • Varsity Scout team:
      Captain, cocaptain, program manager, squad leader, team secretary, Order of the Arrow team representative, librarian, quartermaster, chaplain aide, instructor, or den chief

    • Venturing crew / ship:
      President, vice president, secretary, treasurer, boatswain, boatswain's mate, yeoman, purser, or storekeeper

      NOTE: In order for a Venturer to be an Eagle candidate, he must have achieved the First Class rank as a Boy Scout or Varsity Scout.

  4. Service Project:
    Not completed PRIOR to 18th birthday and/or Eagle Scout board of review
    Must be completed between Life Scout and Eagle Scout board of review dates
Now you may turn in your Eagle Scout Rank Application to your unit leader!
